Duke starts asleep and takes no damage until woken — brew the two poisons from the asylum vats (twelve of each powder plus salt) and apply them to start the fight.

It's a stationary, rhythm-based fight — bring a slash weapon (he's slash-weak), learn the five-tick attack cycle, and it becomes one of the faster DT2 bosses.
